Deposit regulations in connection with the planned visit at Lama Clinic Sp. Z O.O.
§ 1 General Provisions and Definitions
This Deposit Regulation related to a planned consultation or procedure defines the rules for payment, refund, and settlement of the deposit in connection with the reservation of a medical appointment at the healthcare entity “LAMA CLINIC SPÓŁKA Z OGRANICZONĄ ODPOWIEDZIALNOŚCIĄ”, 02-703 Warsaw, ul. Bukowińska 2 lok. U9, NIP: 5214065844, REGON: 528368659, registered in the Register of Healthcare Entities maintained by the Mazovian Voivode, registration book number: 000000275769 (hereinafter referred to as “LAMA CLINIC”).
Capitalized terms used in this Regulation, whether in singular or plural, shall have the following meanings:
Clinic – the healthcare facility operated by the entity “LAMA CLINIC,” providing health services;
Patient – a person requesting or receiving health services from the Clinic, or their representative;
Regulation – this deposit regulation;
Reservation – the date and time of the Patient’s appointment at the Clinic, agreed between the Patient and the Clinic for consultation or other health services;
Party – either the Patient or the Clinic;
Deposit – a monetary amount paid by the Patient to the Clinic as confirmation of the Reservation, paid to the account: LAMA CLINIC SP. Z O.O., Santander Bank Polska S.A. 56 1090 2590 0000 0001 5784 9611
§ 2 Reservation and Its Confirmation. Deposit Payment
To make a Reservation, the Patient contacts the Clinic via:
Failure to meet these conditions may result in cancellation of the Reservation.
§ 3 Deposit Settlement and Refund
If the Patient fails to attend the appointment, the Clinic may retain the Deposit due to organizational and personnel costs incurred.
The Patient will be informed about the decision to retain the Deposit.
The Clinic may set a new Reservation date under the same Deposit if the Patient’s absence is deemed justified.
After two rescheduled appointments and another no-show, the Clinic may retain the Deposit.
Appointment changes must be reported by phone at least 24 hours in advance.
If the appointment is canceled by the Clinic, the Deposit will be refunded in full within 14 days.
§ 4 Cancellation of Reservation
The Patient may cancel the appointment free of charge with the right to a full Deposit refund by notifying the Clinic no later than 24 hours before the appointment.
Cancellations must be submitted in writing via email, SMS, or letter.
§ 5 Additional Provisions
Article 394 of the Civil Code does not apply to Deposit settlements.
Polish law applies to matters not regulated herein, excluding the above-mentioned article.
The Clinic reserves the right to amend this Regulation for important reasons.
In case of conflict with individual arrangements, the agreements between the Parties prevail.
