\u00a7 1 General Provisions and Definitions<\/h4>
  1. This Deposit Regulation related to a planned consultation or procedure defines the rules for payment, refund, and settlement of the deposit in connection with the reservation of a medical appointment at the healthcare entity “LAMA CLINIC SP\u00d3\u0141KA Z OGRANICZON\u0104 ODPOWIEDZIALNO\u015aCI\u0104”<\/strong>, 02-703 Warsaw, ul. Bukowi\u0144ska 2 lok. U9, NIP: 5214065844, REGON: 528368659, registered in the Register of Healthcare Entities maintained by the Mazovian Voivode, registration book number: 000000275769 (hereinafter referred to as “LAMA CLINIC”).<\/li>
  2. Capitalized terms used in this Regulation, whether in singular or plural, shall have the following meanings:
